Individual Coverage $30.30
Insured/spouse $36.20
One-parent Family $42.20
Two-parent Family $49.20

Claims are paid to you the policyholder…Usually within 4 days of claim.

To Enroll: MONTHLY BILLING:

You will need a CREDIT CARD, ATM CARD or VOIDED CHECK when you sign up.
To pay by MONEY ORDER you will need to pay for 3 months of coverage at a time.
